Requisition Forms
An appropriate request form must accompany each sample. As a part of our accreditation guidelines, most information on the request form is
mandatory and the lack of which can lead to delay in sample processing. To avoid this kindly fill out the form legibly and completely.

Medical Form
For all medical tests (except genomics) kindly fill the request form and fill the requested data with the clinical information relevant to
the patient including case history and travel history.

Genomics Forms
Please refer to Genomics Patient Information leaflet for information on genomics testing. Kindly fill Genomics Consent form and
submit together with the Genomics Requisition form for any genomic test.
